Outcomes of Muller’s muscle conjunctival resection With and Without Upper Eyelid Blepharoplasty

Purpose
To compare the preoperative characteristics and surgical outcomes of patients undergoing Muller’s Muscle Conjunctival Resection (MMCR) versus MMCR combined with upper eyelid blepharoplasty.
Methods
A retrospective review was performed of 100 patients (50 in each group) who underwent MMCR or MMCR with concurrent upper eyelid blepharoplasty between January 2024 and December 2025. Recorded variables included age, sex, operated side, preoperative and 3-month postoperative Margin Reflex Distance 1 (MRD1), preoperative levator function (LF), and amount of resection (mm). In patients who underwent bilateral surgery, only one eye was included. Patients with a history of prior eyelid surgery or additional concurrent procedures were excluded. MRD1 was defined as the distance between the upper eyelid margin and the center of the pupil and was measured using a standardized millimeter ruler. Blepharoplasty, when performed, consisted of skin-only excision without removal of muscle or fat tissue.
Results
A total of 100 eyes from 100 patients were included. The MMCR-only group was significantly younger than the MMCR+blepharoplasty group (48.1±15.1 vs. 61.9±11.1 years; p≤0.001). There were no statistically significant differences between the two groups in terms of preoperative MRD1, preoperative levator function, amount of resection, postoperative 3-month MRD1, or change in MRD1 (all p≥0.05). Both groups demonstrated a significant increase in MRD1 following surgery (p≤0.001).
Conclusion
The addition of upper eyelid blepharoplasty to MMCR does not significantly affect eyelid height or surgical outcomes compared with MMCR alone. Concomitant upper eyelid blepharoplasty does not reduce the mean effect of surgery on MRD1. This may assist in preoperative planning for combined procedures.
