Simplifyed medial canthoplasty combined with frontalis suspension for same-time correction of of epicanthus, telecanthus and ptosis in Blepharophimosis Syndrome

Purpose
To evaluate the surgical outcome of epicanthus, telecanthus and ptosis correction using a combined surgery of C-U medial canthoplasty and frontalis suspension with silicon in Blepharophimosis Syndrome.
Methods
We performed a retrospective review of consecutive cases of patients diagnosed with blepharophimosis-ptosis-epicanthus inversus syndrome, who presented to the ophthalmology department of Centro Hospitalar e Universitário de Coimbra, between July 2019 to July 2024. All cases underwent a single time surgery of C-U bilateral medial canthoplasty for correction of epicanthus and telecanthus correction, combined with frontalis sling surgery to correct the co-existing blepharoptosis.
Results
We included 5 cases of BPES, 4 boys and 1 girl, mean age of 2 years (range 0.8 – 18 years). All patients had a positive family history for BPES. After surgery, the epicanthus skin fold was corrected in all cases and surgical scars were unnoticeable after 4 weeks of post op. Ptosis correction was functional and cosmetically successful in all cases. After the follow-up period (range 0.5 – 5.1 years) no reinterventions were necessary.
Conclusion
C-U medial canthoplasty combined with frontalis suspension performed at the same surgical time in Blepharophimosis Syndrome was found to be an effective procedure in the correction of epicanthus, telecanthus and ptosis with good funtional and aesthetic results.
