Purpose
To propose a surgical approach for the reconstruction of an anophthalmic socket with a history of multiple prior interventions and radiation therapy, presenting with a retracted palpebral fissure and absence of both superior and inferior fornices.
Methods
To perform the reconstruction of the anophthalmic socket, the procedure begins with the opening of the palpebral fissure, followed by layered dissection to create a pocket and define the superior and inferior fornices that will receive the graft. A full-thickness skin graft is harvested from the patient’s inner upper arm and used to cover a socket conformer, with the epidermal surface oriented inward. The conformer is then positioned conventionally within the newly created socket. The epidermis of the skin graft is sutured to the newly formed palpebral margins, and the palpebral fissure is closed. One month later, the fissure is reopened to accommodate the ocular prosthesis.
Results
Despite mild palpebral retraction observed one month after surgery, the reconstruction yielded a satisfactory aesthetic outcome and enabled the patient to wear the ocular prosthesis successfully.
Conclusion
The use of a socket conformer covered with a full-thickness skin graft has shown to provide satisfactory postoperative aesthetic outcomes in patients with a history of multiple prior socket reconstructions. However, long-term results remain to be determined, and extended follow-up is required to assess the durability of this technique.
