The impact of blepharoplasty on patient’s self esteem

Purpose
This study aimed to explore how blepharoplasty impacts patients’ self-esteem using the Rosenberg Self-Esteem Scale. It also assesed the post-operative satisfaction and the frequency of common complications that may arise after the surgery.
Methods
We conducted a cross-sectional retrospective study at King Khaled Eye Specialist Hospital from January to April 2025. The study involved 183 patients who had undergone upper and/or lower eyelid blepharoplasty between 2017 and 2024. Data collection was carried out through structured interviews led by physicians, utilizing a detailed questionnaire that gathered demographic details, surgical specifics, pre-and post-operative satisfaction, self-esteem levels, and post-operative complications. Statistical analysis was performed by using SPSS.
Results
The total sample size is 183 respondents, of whom 21.9% are males and 78.1% are females . 64.5% operated on upper eyelids only, 18.6% lower eyelids only, and 16.9% operated on both upper and lower eyelids.After the surgery, patient satisfaction showed a marked increase, with 79.8% reporting high levels of satisfaction compared to just 37.7% before the blepharoplasty. Self-esteem levels were generally high both before and after surgery, with mean scores of 3.56 (±0.89) pre-op and 3.50 (±0.91) post-op. However, a small but statistically significant decrease was observed post-surgery (Wilcoxon Z = 2.669, p = 0.008). Interestingly, 42.1% of patients reported high self-esteem before surgery, while 38.3% reported high self-esteem after surgery. Regarding complications, 51.9% of patients reported experiencing post-operative complications, with dryness and irritation being the most common. The questionnaire demonstrated strong internal consistency and reliability.
Conclusion
Blepharoplasty improves patient satisfaction regarding their eye appearance. Although self-esteem levels were high both before and after the surgery, a subtle decline was noted post-operatively, suggesting that psychological outcomes may be influenced by more than cosmetic results alone.
