Operative results of bilateral Muller muscle conjunctival resection (MMCR) with prolene or vicrly depending on suture type

Purpose
The aim of this study was to evaluate the operative results, complications and revision surgeries of patients with bilateral MMCR using 6/0 prolene and MMCR+Blepharoplasty using 6/0 vicrly.
Methods
Between 2020-2023, bilateral MMCR prolene and bilateral MMCR+Blepharoplasty vicrly were evaluated and compared in terms of margin reflex distance (MRD-1), levator function, lagophthalmos, complications and revision surgery.The study was performed as a retrospective file review.
Results
64 eyes of 32 patients were included in the study. Of the 32 patients, 16 (%50) were female and 16 (%50) were male. There were equal numbers of men and women in the groups. Those made with prolene were defined as group 1 and those made with vicrly were defined as group 2. In group 1, the mean right eye (OD) MRD-1 was 1.37±0.15 preoperatively and in group 2, the mean OD MRD-1 was 2.00±0.18 preoperatively. There was no statistical difference between the two groups preoperatively. In group 1, the mean right eye (OD) MRD-1 was 3.31±0.17 in response to preoperative phenylephrine and in group 2, the mean OD MRD-1 was 3.75±0.11 in response to preoperative phenylephrine. In group 1, the mean right eye (OD) MRD-1 was 3.25±0.19 in postoperative 1.month and in group 2, the mean OD MRD-1 was 3.43±0.18 in postoperative 1.month. In group 1, the mean right eye (OD) MRD-1 was 3.25±0.17 in postoperative 3.month and in group 2, the mean OD MRD-1 was 3.56±0.22 in postoperative 3.month. Similar results were obtained for the left eye (OS) in the preoperative and postoperative periods. There was a significant difference between preoperative and postoperative MRD-1 in both groups. Preoperative phenylephrine response and postoperative MRD-1 values were similar and there was no significant difference. Levator surgery was performed in 1 patient in group 1 and 2 patients in group 2 as revision surgery.
Conclusion
Bilateral MMCR is a highly successful surgery in patients who respond to phenylephrine, and MMCR is performed according to the amount of response. There is no difference between suture choices in terms of surgical success. Large series studies are needed.
