Comparative Analysis of Tranexamic Acid and Local Anesthetics in Hematoma Prevention After Upper Blepharoplasty

Purpose

Hematoma formation is a known complication following upper blepharoplasty, potentially compromising surgical outcomes and prolonging recovery. This study aimed to compare the efficacy of tranexamic acid (TXA) and local anesthetics with vasoconstrictors in reducing the incidence of postoperative hematoma in patients undergoing upper blepharoplasty.


Methods

A total of 60 patients (40 females, 20 males; mean age 57.3 ± 6.4 years) scheduled for upper blepharoplasty were divided into two groups. Both groups received local anesthetic infiltration with 1.5 mL of lidocaine 2% with epinephrine 1:100,000 or bupivacaine 0.5% with epinephrine 1:200,000, administered along the supratarsal crease and lateral canthus with a 5-minute waiting period for optimal effect. In addition, Group 1 received 1 mL of tranexamic acid (100 mg/mL) via local infiltration at the same sites, while Group 2 received local anesthetics only. Postoperative hematoma incidence was assessed clinically at 24 hours, 48 hours, and 7 days after surgery.


Results

The results showed a significantly lower incidence of hematoma in the TXA group (5%) compared to the local anesthetic-only group (13%). TXA demonstrated superior efficacy in reducing postoperative bleeding, particularly in patients with known vascular fragility.


Conclusion

These findings suggest that the use of tranexamic acid in combination with local anesthetics may enhance hemostatic control in upper blepharoplasty, reducing hematoma formation without significant side effects. Further research with larger cohorts is needed to confirm these results and explore the optimal TXA dosage and administration techniques for oculoplastic surgery.
