Use of Apple Vision Pro as a Support Tool for External Dacryocystorhinostomy: Case Report and Remote Tutoring Feasibility

Purpose
Augmented reality (AR) in the operating room can overlay radiologic images and textual guidance without breaking sterility. We describe external dacryocystorhinostomy (DCR) supported by Apple Vision Pro, assessing feasibility, advantages, and current limitations.
Methods
A 58-year-old female with chronic nasolacrimal duct obstruction underwent external DCR. The primary surgeon used an Apple Vision Pro headset to:- Overlay maxillofacial CT slices via a DICOM viewer- Access operative notes and textbook PDFs hands-free- Continuously record the procedure through the headset’s front camera- Illuminate the surgical cavity with a headset-mounted LED module- Apply real-time digital zoom via gesture control- Enable remote tutoring by a senior surgeon over the integrated video call
Results
The 45-minute procedure was complication-free. Sterility was maintained; the surgeon accessed all virtual tools seamlessly. The remote tutor provided live feedback, confirming distance mentoring feasibility. The patient was discharged same-day and remained symptom-free at one-month follow-up.
Conclusion
Apple Vision Pro can safely support external DCR, enhancing data accessibility and enabling real-time remote tutoring. This technology shows promise for improving surgical outcomes through enhanced visualization and expert guidance, particularly in settings with limited access to specialists. Future hardware refinements—especially integrated illumination and improved optical zoom—will further optimize its utility.
