Surgical Outcome of upper and lower lid retraction corrected by Novel Filamentous Knitted Polyester Fabric in thyroid eye disease

Purpose

To evaluate the functional and cosmetic outcomes after using Novel Filamentous knitted polyester fabric to correct upper and lower lid retraction in Thyroid eye disease
To evaluate the safety, efficacy, and durability of the FKPF


Methods

A prospective interventional study was done on 14 eyes. This surgical procedure introduces Novel FKPF over the lid as spacer material to correct upper and lower lid retraction in TED. LPS, mullers muscle recessed from tarsal plate and create a semilunar gap where FKPF is fixed as spacer material to prevent LPS reattachment. Lifting of the upper lid by intact lateral and medial horn LPS.In the lower lid through the conjunctival approach inferior retractor is detached from the tarsal plate creating a semilunar gap where FKPF is introduced as spacer material . INCLUSION CRITERIA: All patients with Thyroid Eye Disease having upper and lower lid retraction. Minimum scleral show (2mm) in upper and lower lid.Limited proptosis 23mm,Significant squint and restriction of ocular movement,Patients with TED having exposure keratitis due to upper and lower lid retraction. Systemic skin disorders like Steven-Johnson syndrome, widespread psoriasis, Active thyroid disease


Results

In this study, sixteen eye operated all are upper and lower lid retraction corrected by FKPF.In this study, pre-op mean MRD-1 values with sd was 6.21±1.2mm and post-op it was 4.14±1.2mm. Pre-op MRD-2 values with sd was 7.03±1.2mm and post-op it was 4.57±0.48mm.Pre-op mean PFH value with sd was 14.03±2.03mm and post op it was 10.03±0.45mm.Pre-op mean LPS action with sd 18.42±2.37mm and post op it was 12.92±0.73mm.Pre op mean sclera shows upper lid with sd 2.82±1.2mm and post-op it is nil. Pre-op mean scleral shows with sd 3.14±1.5mm and post it is 0.21±0.80mm.


Conclusion

FKPF is inert, non-reacting, tissue friendly. It is cost-effective and easily available in our hospital setup.ALL lid retractions are corrected, cosmetically anatomically, and physiologically. All the patients are mentally highly satisfied
