Buttock dermis-fat graft as a secondary orbital implant in a child-case report

Purpose
To stress the value of buttock dermis-fat graft as a secondary orbital implant in severe anophthalmic socket syndrome following an early primary silicone implant extrusion due to MRSA infection in the child after intraarterial chemotherapy and thermotherapy of retinoblastoma.
Methods
Three-year-old boy has undergone 17 cycles of intraarterial chemotherapy and thermotherapy due to relapses of retinoblastoma in the right eye. Unfortunately, the third retinoblastoma relapse prompted secondary enucleation. The surgery with primary silicone orbital implant placement was uneventful. Only six days later spontaneous extrusion of the silicone orbital implant ensued. The orbital swab after the implant extrusion revealed a Methicillin-resistant Staphylococcus aureus (MRSA) infection. Due to the long duration of systemic and local treatment complicated by the recurrence of the infection, the boy developed severe right-side anophthalmic socket syndrome. The reconstructive surgery with dermis-fat graft was planned after the negative swab of the orbit for MRSA was obtained. The size of the fat component dictated the harvesting area. In cooperation with a pediatric surgeon, a dermis-fat graft of 25 mm in diameter was harvested from the right-side buttock.
Results
The surgery was uneventful. No complication of the harvesting site was reported. The sutures were taken out 12 days postoperatively. Dermis epithelized in 5 weeks and the child got an eye prosthesis.
Conclusion
Buttock dermis-fat graft proved to be an irreplaceable material for socket contracture reconstruction following MRSA infection in a child with retinoblastoma treated with chemotherapy and thermotherapy. It is the only material that combines both volume and conjunctival replacement.
