Histopathological Analysis of Levator Muscle Fibers in Patients with Lid Elevation during Ipsilateral Abduction and Patients with Jaw Winking Phenomenon

Purpose
Marcus Gunn (jaw winking) phenomenon (MGP) is a form of congenital ptosis characterized by misfiring of the eyelid associated with movement of the jaw. Based on previous histopathological analysis, it is believed to represent a neurogenic form of atrophy.
Methods
We report the histopathological and clinical features of MGP in comparison to similar conditions of upper eyelid misfiring in a retrospective study of congenital ptosis cases with MGP who have undergone surgical repair in 2 tertiary care hospitals over a period of 12 years to identify cases of MGP and compare their histopathological findings to 2 cases with upper eyelid misfiring in association with extra ocular movements.
Results
Amongst 136 patients of congenital ptosis with MGP, 11 (8%) cases of MGP or misfiring were identified. Of the 11 cases 9 (6.6%) were of typical known MGP and 2 (1.4%) were of eyelid misfiring similar to MGP. The histopathological changes in all cases were similar, and shared findings that were consistent with a neurogenic type of muscle atrophy.
Conclusion
We have concluded that MGP and similar misfiring conditions with synkinesis have variable clinical presentations but share similar histopathological findings. This suggests a common underlying etiology.
