Complex anophthalmic socket customised reconstruction: a case series.

Purpose

To describe a series of complex socket reconstructions in patients who had previously undergone multiple failed surgical procedures.


Methods

A total of four patients with a past history of evisceration/ enucleation secondary to trauma and who had previously undergone a minimum of 5 surgical procedures to treat Anophthalmic Socket Syndrome were included. All of them presented with recurrent partial extrusion of the primary/ secondary orbital implant. The initial objetive was to preserve the original orbital implant in place if possible, and to provide volume to the socket using autologous tissue, as some of them had presented recurrent rejection to heterologous materials.


Results

Three patients underwent total reconstruction of the socket using a 3-layer-autologous graft from their thighs keeping the original implant in place; one of them being left without any implant as he had removed the primary one by himself prior to surgery. Amniotic membrane was as well used to avoid contraction of the inferior fornix. The fourth patient had a previous history of caustication and multiple failed volume restoration procedures, and an additional orbital floor implant was required to avoid enophthalmos. Avoiding post surgical infection in such cases remained a challenge.


Conclusion

Patients who present with a typical Anophthalmic Socket Syndrome may benefit from secondary orbital implant or reconstruction surgery, but the surgical procedure is more challenging and the results are less predictable than primary orbital implantation. Patients undergoing secondary orbital implantation are more likely to require additional surgical procedures. We herein report a combination of patient-customised procedures which aim is to successfully restore volume deficiency in a single surgical procedure.
