Purpose
A Trabut lid plate is modified as an all-in-one instrument to facilitate exposure during transconjunctival surgery of upper or lower eyelid malpositions.
Methods
Desmarres lid retractor replaces one side of a Trabut plate, the other side of the plate remaining flat. Angles are added between these two sides and the body of the Trabut plate to fit both the upper eyelid and the eyebrow arch. The main interest of this new design is to expose the eyelids by stretching the traction sutures over the Trabut plate, avoiding any need for assistance and freeing the surgical site.
Results
Each side of the instrument is used depending on the tissue to be exposed : -The Desmarres side is aimed at upper eyelid malpositions as ptosis or retractionby allowing Müller’s muscle transconjunctival surgery. -The flat side allows access to the upper tarsus, for shortening it during ptosis surgery or for harvesting it during an eyelid repair. The flat side allows also access to the lower eyelid retractors during ectropionand entropion surgery.
Conclusion
This all-in-one lid plate can be used daily to facilitate exposure during transconjunctival surgery of upper or lower eyelid malpositions, avoiding any need for assistance during this step.
