Dose-response effect on scar treatment with intralesional serial 5-fluorouracil injections

Purpose
The beneficial effects of antimetabolite injections on skin and conjunctival scar reduction have been published. This study’s author previously established a positive correlation between patients’ satisfaction and positive clinical outcomes. Less known is, however; the dose or quantity-response effect of 5FU on main scar features
Methods
An interventional, prospective case series including 12 patients treated with 1 to 7 5FU injections at two health facilities was conducted. Photos and clinical notes before and after were taken to evaluate the dose-response effect after each application. Scar hypertrophy, redness, thickness, and indentation were evaluated. Friedman test was performed for each variable separately to test if the mean variances were equal throughout all groups. A post hoc and Wilcoxon Signed Rank tests with Bonferroni correction were performed to test the difference in-between groups with α=0.01
Results
Friedman test for scar hypertrophy, redness, indentation, and thickness showed significant differences between different time measurements of injections (p<0.001, p<0.001, p=0.018, p<0.001, respectively). In-between groups Wilcoxon analysis yielded a difference between pre-injection and 1 injection for hypertrophy, redness, thickness, and scar indentation p=0.025, p=0.008, p=0.157, p=0.025, respectively. Between pre-injection vs 2 injections p=0.002, p=0.004, p=0.005, p=0.038, respectively. Between pre-injections vs 3 and more injections p=0.006, p=0.007, p=0.01, and p=0.059, respectively. The difference between 2 and 3 or more injections was p=0.011, p=0.007, p=0.007, p=0.083
Conclusion
Results indicate there are significant differences in hypertrophy in patients based on the injection number for two or more injections, redness for one or more, and thickness for two or more injections
