Thyroid Eye Disease (TED) in the pediatric population: A retrospective study at a Tertiary Eye Care Institute

Purpose
To study the demographics, risk factors, clinical presentations, and management of Pediatric TED at a tertiary eye care center
Methods
This is a retrospective observational study from a tertiary eye care institute in south India. We reviewed the medical records of all the patients aged 18 years or younger who were clinically diagnosed with TED between the year 2013 to 2018. Clinical history, ophthalmic examination, and management were recorded
Results
Fifteen patients with TED were included; 10 were females. The mean age at presentation was 14.26 years (Range: 8-18), and 20% of had a family history of systemic thyroid disease. Euthyroidism was noted in 8 patients (50.33 %), and Hyperthyroidism in 2 patients (13%). Bilateral disease was observed in 70.33% of the patients. Proptosis was the commonest presenting symptom, noted in 9 out of 15 patients. The most common clinical sign was upper and lower lid retraction which was observed in 80% of our patients followed by proptosis (33.33%). The mean Hertel’s exophthalmometry reading was 19.25 (SD: 2.42 Range: 16-25). At presentation, 60% of patients were active based upon timeline (≤12 months since onset), and average clinical activity score at presentation was 0. Of the 15, 13 (86.66 %) were managed conservatively, 1 underwent bilateral orbital decompression, and 1 underwent lower eyelid retractor release procedure. The mean follow up was 15.58 months (Range 1-84). None of our patients had optic neuropathy or strabismus.
Conclusion
Pediatric TED shows female preponderance, and bilaterality. Euthyroid status is noted in 50% cases, and clinical activity score is low. Thyroid eye disease is less severe in the pediatric population and the majority of the cases can be managed conservatively.
