Nizetic technique: rotation of the tarsus in advanced trachoma

Purpose

Trachoma infection causes scarring of the conjunctiva, producing trichiasis and entropion. It is the main cause of blindness in underdeveloped countries. Due to globalization, we find cases of advanced trachoma in our environment, with several interventions performed and serious involvement of the tarsus.To resolve these alterations, we must resort to ancient techniques.


Methods

78-year-old man from Morocco who suffered trachoma in his childhood, and underwent surgery several times in France and in our country. It is a single eye, which has suffered two fungal ulcers. He sees light perception, with significant entropion of the upper eyelid, the chamber is formed, corneal leukoma, phacosclerosis and glaucoma treated with antiglaucomatous drugs. He has had a spontaneous corneal perforation treated with multilayer amniotic membrane graft. Because it is a single eye, with the interventions previously performed with recurrence and to avoid a new infection and perforation of that eye, we perform the Nizetic technique.


Results

The Nizetic technique is a variant of the Blascovics technique that consists of total rotation of the tarsus, turning it 180º: the tarsus is freed from the levator on the anterior surface and from the conjunctiva and Muller’s muscle on the convex side of the eyelid except for 5 mm from the center, the tarsus is rotated 180º, leaving the anterior surface in an anterior position. We performed this intervention on the left eye of our patient.


Conclusion

In patients with serious pathologies, we must recover old procedures that can solve therapeutic problems. This technique on the tarsus and conjunctiva controls the elements that intervene in the pathogenesis of cicatricial entropion in an adequate way, such as surgery on the tarsus.
