Primary squamous cell carcinoma of the orbit

Purpose

Primary squamous carcinoma (SCC) of the orbit is rare. The most common treatment is surgical excision of the tumor or orbital exanteration, with follow-up of the patient. Its therapy is not standardized because there are few cases.


Methods

An 89-year-old male patient consulted for a tumor of the left superonasal orbit of two weeks of evolution. On examination a hard lesion was palpated, with slight pain on pressure; the patient evidenced a rapid growth of the lesion; a computed tomography (TC) scan was performed showing a 20 x 14 mm extraconal tumor affecting the greater oblique muscle. Ophthalmologic examination showed pseudophakia, corneal edema, macular atrophy, and glaucoma under treatment in both eyes. Visual acuity is 0,05 in both eyes. A biopsy of the tumor was performed and the result was SCC. Abdominal thoracic TC was performed without tumor alterations and a new orbital CT showed lesion growth of 10mm in 3 weeks and also involvement of the superior rectus muscle. The Tumor Committee ruled out medical treatment with Cemiplimab.


Results

Due to the rapid growth of the tumor, the location and without mestasis, it was decided to intervene, in agreement with the patient, performing orbital exanteration with temporal muscle flap. The anatomopathologic result was SCC with a border zone to the superonasal periosteum, so the tumor committee advised 30 sessions of radiotherapy.


Conclusion

Epidermoid carcinomas are fast growing, aggressive and add another element to the challenge of the therapeutic decision due to the orbital location. This pathology must be studied by an interdisciplinary team in order to achieve the best possible outcome for each patient.
