Bilateral eyelid erosions: an uncommon ocular manifestation of pemphigus vulgaris

Purpose
To report a case of pemphigus vulgaris with bilateral eyelid erosion

Results
A 55-year-old patient with no medical history who was admitted in the dermatology department for the evaluation of cutaneous lesions involving the face, hands, the back, abdomen, and inferior members. She was referred to our department for the extension of the lesions to the periorbital region. The examination found a best corrected visual acuity of 20/20 in both eyes. The eyelids were the site of post bullous lesions with squamous epidermal collarettes. The lesions spared the eyelid margins. The conjunctiva was white, the anterior segment and the fundus examination were normal in both eyes. A skin biopsy with direct immunofluorescence was performed confirming the diagnosis of pemphigus vulgaris. Systemic corticosteroids with an initial dose of 1.5mg/kg/day and 150mg/day of azathioprine were given to the patient by dermatologists. The outcome at one-month follow-up was the progressive cicatrisation of most lesions of the skin, the eyelid lesions were slower to heal.
Conclusion
Most of the autoimmune bullous diseases induce cicatrising conjunctivitis with symblepharon. The most common ocular manifestations of pemphigus vulgaris are no cicatrising conjunctivitis and lid margin erosion. Eyelid erosion without involvement of the conjunctiva or lid margins is a possible manifestation.
