Pyoderma gangrenosum of the eyelid : A challenging case report

Purpose

Pyoderma gangrenosum (PG) affecting the eyelid is extremely unusual and it might mimic another differential diagnosis. Timely diagnosis and proper management are imperative to prevent undesirable eyelid disfigurement and challenging reconstructive surgery in the future.This case report shed the light on clinical diagnosis and management of pyoderma gangrenosum related eyelid lesions.


Methods

One patient with eyelid pyoderma gangrenosum is presented with a literary summary


Results

29-year-old male known to have ulcerative colitis who developed a non-healing ulcerative lesion on his left upper eyelid which did not respond to conventional chalazion treatment. A clinical diagnosis was made along with an incisional biopsy and treatment with Intralesional triamcinolone was instituted but with a limited response. Propitiously, controlling the underlying disease by initiating immunomodulator therapy successfully halted the progression of the eyelid lesion, and eliminated the need for any further eyelid interventions.


Conclusion

Pyoderma gangrenosum is a rare and aggressive condition that should be considered in the differential diagnosis of any ulcerative lesion of the eyelid. Due to its challenges in diagnosis, treatment should be targeted toward controlling the underlying systemic disease in collaboration with a gastroenterologist. Early diagnosis and management are crucial to prevent facial disfigurement and the need for complex eyelid reconstruction in the future.
