Latteral scar dehiscence after upper blepharoplasty

Purpose
Upper eyelid blepharoplasty, a common cosmetic surgical procedure, aims to enhance the appearance of the upper eyelids. While generally safe and effective, it is not without potential complications. Lateral scar dehiscence is a relatively uncommon but recognized complication after upper eyelid blepharoplasty. Studies report varying incidence rates, around 1-2% of cases. Factors contributing to this complication include the technique used, patient’s skin type, and postoperative care.
Methods
A retrospective study was performed to determine frequency of lateral scar dehiscence in patients who undergone upper eyelid blepharoplasty in 2022/2023 ( 404 cases) with only skin closure compared to the group of 396 cases in 2023/2024 who had 2 additional tension-relieving subcutaneous buried sutures placed in the lateral part of the wound. The surgery was performed by one surgeon with the same sutures and operation technique.
Results
Lateral scar dehiscence was statistically significantly higher in younger female group and in cases with just skin closure. The additional sutures significantly decreased lateral scar dehiscence occurrence especially in younger females. In older group no lateral scar dehiscence was observed.
Conclusion
As there was a statistically significant difference in the frequency of lateral scar dehiscence between the study groups, it seems recommended, especially in younger patients , to use additional subcutaneous buried sutures in the lateral part of the wound
