Effectiveness of External Dacryocystorhinostomy in Soft Lacrimal Probing and Revision Surgery Cases

Purpose
This study aims to evaluate the outcomes of external dacryocystorhinostomy with osteotomy using Arruga’s trephine or an ultrasonic tip in patients with a soft stop on lacrimal probing and previous unsuccessful lacrimal surgeries.
Methods
A retrospective analysis was conducted on external dacryocystorhinostomy cases performed between January 2022 and 2024 at a tertiary hospital. The study included patients with nasolacrimal duct obstruction, a soft stop on lacrimal probing, or prior unsuccessful lacrimal surgeries. Cases not meeting these criteria were excluded.
Results
A total of 63 eyes underwent surgery (5 patients had bilateral procedures), including 15 men and 43 women, with an average age of 69.39 years and a mode of 70 years. Osteotomy was performed using Arruga’s trephine in 42 eyes and an ultrasonic tip in 21 eyes. The minimum follow-up period was 3 months.
Epiphora was resolved in 87.3% of the 63 operated lacrimal ducts. Twenty-one eyes had a history of previous unsuccessful lacrimal surgeries. At a significance level of 5%, no significant difference was found between groups with and without previous infections or between primary and secondary lacrimal surgeries. Additionally, no significant difference was observed between surgeries performed with Arruga’s trephine and the ultrasonic tip. While evaluating surgeon categories against outcomes, no significant differences were observed; however, the p-value (0.111) approached significance, suggesting a potential trend.
Conclusion
These results reinforce that external dacryocystorhinostomy is one of the preferred techniques for treating nasolacrimal duct obstruction. Prior infections or surgeries did not significantly impact outcomes.
The p-value (0.111) in evaluating the association between outcomes and surgeon category, although not statistically significant, suggests a potential trend and highlights the importance of surgeon training. The inherent limitations of a retrospective study should be considered when interpreting these findings.
