Bilateral spheno-orbital meningioma: a rare entity? Ten new cases.

Purpose
To define bilateral spheno-orbital meningioma (SOM) and describe clinical and radiological features.
Methods
We defined bilateral SOM as: bilateral and per side contiguous (1) dural involvement located at the sphenoid ridge, and (2) hyperostosis of the sphenoid bone and (3) orbital involvement. The electronic files of all patients known with SOM seen at the multidisciplinary outpatient clinic in our tertiary referral hospital were reviewed. Magnetic Resonance and Computed Tomography images were evaluated by a Head and Neck radiologist, an experienced orbital surgeon and a neurosurgeon.
Results
127 patient files were reviewed and after imaging evaluation ten patients with bilateral SOM were identified. All patients were female with an age at diagnosis between 41 and 62 years (average 49.6). One patient was diagnosed with neurofibromatosis type 2 and two patients with meningiomatosis. There was a history of depression in three patients and three others were intellectually disabled. All patients had a history of contraceptive medication and seven out of ten patients had a history of injectable medroxyprogesterone usage. There was compressive optic neuropathy in 10 out of 20 eyes, bilateral in four patients. Four patients underwent bilateral surgery and three patients unilateral surgery. Two patients needed radiotherapy. Average follow-up was 6.6 (3-13) years and visual function was stable in 13 out of 20 eyes, deteriorated in two, and improved in five eyes.
Conclusion
Bilateral SOM might not be as rare as reported in literature. There is a possible relationship with use of progestins. Treatment strategy and indications for simultaneous bilateral surgery depend on the presence of optic neuropathy and clinical features.
