Title: Case of Silicone rod exposure with pseudomonas aeruginosa infection treated with ciprofloxacin and tube excision and frontalis flap

Purpose

To dicusss the management of silicon tube exposure and chronic infected granuloma with pseudomonas aeruginosa


Methods

A case of a 37-year-old Bahraini gentleman who presented with 8 months history of a forehead infected granuloma after silicon rod brow suspension ptosis repair done in a different center. Culture swabs were taken which showed pseudomonas infection.
He was started on Oral ciprofloxacin 500mg TDS but was not improving due to silicon rod exposure from forehead skin.

He underwent granuloma and silicon rod excision and redo ptosis repair with frontalis flap. Care was taken not to injury the temporal branch of the facial nerve. He continued the Oral Ciprofloxacin 3 weeks.


Results

He presented 2 weeks post-surgery. His wound healed well and there was no sign of infection. MRD1 was 3 mm. There was no lagophthalmos. (before and after photos available)


Conclusion

Silicon tube brow suspension can be complicated but tube exposure. This patient presented with pseudomonas aeruginosa infection. Culture and sensitivity showed sensitive to Ciprofloxacin antibiotics. Patient was started on the treatment but there was no improvement. A decision to excise the silicon tube was made and a frontalis flap was done. Patient was happy with his surgical outcome.
Frontalis flap is an option for repair of congenital ptosis and can be recommended for patient who present with complication of silicon tube brow suspension complications.
