Management of a major upper eyelid reconstruction: our experience

Purpose

Reconstruction of a large and full thickness upper eyelid defects is, nowadays, an interesting and controversial challenge. Several surgical techniques have already been discussed achieving to a satisfactory function and aesthetic immediately postoperative results. Different outcomes were reported in a medium and long term follow up, resulting in ophthalmologist and plastic complications. Here, we report our experience of management of major upper eyelid reconstruction after tumor resection.


Methods

Two patients were referred to our ophthalmoplastic outpatient in Conegliano – Vittorio Veneto Hospital with a bulky and ulcerated mass of the upper eyelid. In both cases the evolution was dramatically rapid and compromised upper eyelid function. Incisional biopsy was promptly performed and revealed a malignant tumor in either case. All surgical procedures were conducted under local anesthesia. An immediate reconstruction was planned, after negative results of the extemporaneous histopathological examination.


Results

Both cases required extensive resection of the mass resulting in a large upper eyelid defect.The following reconstructive surgical steps allowed to achieve our planned results: free tarsal graft of defect size was excised from the other upper eyelid to reconstruct posterior lamella; elevator muscle was fixed on tarsal plate; a pre septum orbicular muscle and skin flap was transposed to cover the tarsal graft. Upper eyelid reconstruction completed with a wolf skin graft.


Conclusion

An accurate and detailed planning of the upper eyelid reconstruction allowed to achieve a satisfactory morphological and functional outcome.
