Eyelid basal cell carcinoma in the elderly: A case study of effective surgical management and reconstruction

Purpose
To present the management of a case diagnosed with basal cell carcinoma (BCC) affecting both the upper and lower eyelids of the left eye in an elderly patient and to point out the full recovery after reconstruction of the area.
Methods
An 84-year-old male patient with no previous history of malignancies presented to our clinic in January. The carcinoma on the upper eyelid had appeared approximately 3 months prior and had rapidly enlarged, spreading to the canthus margin and the lower eyelid. Surgical excision was performed with a 2 mm clear margin. The excised tumor was sent for histological analysis, and subsequent reconstruction was achieved using a skin graft from the subclavicular area.
Results
Basal cell carcinoma, the most common type of skin cancer, is known for its slow growth and low metastatic potential but can cause significant local destruction if not treated promptly. Differential diagnoses for BCC include squamous cell carcinoma, sebaceous gland carcinoma, and benign lesions such as chalazion or keratoacanthoma. Histological analysis confirmed the diagnosis of BCC. Post-operative management involved referral to an oncologist, who found no further malignancies. The patient achieved full recovery with no recurrence noted during follow-up.
Conclusion
Effective management of basal cell carcinoma of the eyelid involves prompt surgical excision with clear margins and appropriate reconstruction to preserve function and appearance. Continuous follow-up is essential to monitor for any signs of recurrence. Multidisciplinary coordination among ophthalmologists, dermatologists, and oncologists ensures comprehensive care and optimal outcomes.
