Purpose

To demonstrate the surgical techniques and advantages of posterior white line approach in ptosis correction.

Results

This presentation demonstrates the surgical technique of posterior white line approach in ptosis correction and its uses for variety of etiologies. Posterior white line approach is suitable for patients with moderate to good levator function and irrespective of the response to phenylephrine test. It can be used to correct various degrees of ptosis. This surgical technique is used for congenital ptosis, involutional ptosis, cosmetic ptosis, traumatic ptosis and iatrogenic ptosis, which we demonstrate in this presentation. Postoperative outcomes were satisfactory. It is especially useful in East Asians as it gives a good lid crease and one could circumvent the unique anterior anatomy of an East Asian eyelid. Complications include granuloma, notching and corneal abrasion. However, all these complications were minimal and resolved spontaneously over time.


Conclusion

Posterior white line approach is a simple, direct and safe surgical technique for ptosis correction. It is a faster procedure with a more predictable outcome, gives good contour and symmetry, has a shorter recovery time and excellent success rate in comparison to anterior approach. Furthermore, this surgical technique does not require skin incision, hence, it is a scarless and minimally invasive procedure for ptosis correction.
