MIDFACE LIFTING WITH MESOTHREADS

Purpose
Mesothreads are biodegradable, biocompatible and safe lifting threads composed of pressed vicryl, providing them with high strength and tensile strength. In patients with positional anomalies of the lower eyelids, the presence of a negative vector should always be sought, as this determines the approach in their treatment. Mesothread lifting is a less invasive procedure than the surgical approach. A cannula is used, which does not traumatize the tissues, but layers them, which is why the postoperative period runs more smoothly and with fewer complications. We aim to present a case series of temporary midface lifting with mesothreads in patients with a positional anomaly of the lower eyelid and the presence of a negative vector, in which conventional surgery cannot be performed or have to be postponed.
Methods
A case series of 10 retrospective patients with negative vector and positional anomalies were followed for 2 years. All patients had midface lifting done with mesothreads. Hypercorrection is always done because during the healing process, the tissues of the face relax slightly downwards.
Results
In all patient, we obtained good functional and satisfactory cosmetic results. In all cases, the main complaint was determined by the available entropion (7 cases) and ectropion (3 cases). A classical operation for entropion/ectropion in the presence of a negative vector would lead to the deflation of the lower eyelid under the bulb in the direction of the lower orbital rim, which would aggravate the patient’s complaints.
Conclusion
PDO mesothreads are a good option for correcting the negative vector of the lower eyelids with an extremely short and gentle surgical intervention. We expected the effect to be short-lived, as cosmetic surgery has proven, but for 2 years, the mesothreads maintain the position of the lower eyelid.
