MÜLLER’S MUSCLE CONJUNCTIVAL RESECTION: UPPER EYELID CONTOUR CHANGES AFTER THE TRADITIONAL CLOSED APPROACH VERSUS THE OPEN-SKY TECHNIQUE WITH AN INTERNAL RUNNING SUTURE

Purpose

To perform a quantitative comparison of the upper eyelid contour changes after traditional Müller’s muscle conjunctival resection (MMCR) and an open-sky technique with an internal running suture (OST).


Methods

Comparative cross-sectional analysis of the pre- and postoperative contours of two groups of upper lids that underwent MMCR (n = 19) and OST (n = 49). The Putterman’s clamp was employed to lift the posterior lamella in all MMCR procedures. In the OST technique, Müller’s muscle was dissected away from the aponeurosis until the white line was just visualized. In both techniques, the amount of posterior lamella resection was consistent, typically 8 mm. Using Image J software, 4 medial and 4 lateral margin lid distances were measured on Bézier lines representing the pre- and postoperative lid contours. All procedures were performed under local anesthesia with intravenous sedation. Intraoperative contour adjustment was not performed in any procedure. A two-way (group, position) analysis was employed to compare the contour changes


Results

No differences were detected between the techniques in the mean increase of the midpupil, lateral, and medial lid distances


Conclusion

The open-sky technique of Müller’s muscle resection with a single running suture, without any contour adjustment, provides the same contour changes as those induced by the traditional closed approach with the Putterman’s clamp.
