Three approaches to evisceration : a comparative clinical review

Purpose
To compare the outcomes of three different evisceration surgeries.
Methods
We retrospectively reviewed clinical charts of all patients who underwent an evisceration surgery and orbital implantation at our center between 2018 and 2021. Patients were included in three differents groups based on the surgical technique used : evisceration performed with the creation of four flaps with a release from optic nerve attachment, evisceration with two flaps and “on the table” evisceration, where an enucleation was first performed, followed by an evisceration with autologous sclera wrapping on the anterior surface of the implant.
Results
A total of 110 patients were included, with an mean age of 35 years and an average follow up of 23 months. Fifty patients were included in the four-flaps group, 14 in the two-flap group and 45 in the on-the-table group. The mean implant size was 20 mm (ranging from 16 to 22 mm), with no differences between our three groups. There was no difference in analgesic intake. Mobility and aesthetics were deemed satisfactory in most of the cases. There were no statistical differences between the groups in terms of complications, as only four patients (3.6%) were diagnosed with implant exposure (two in the four-flaps group and two in the on-the-table group). About one third of our patients (31) underwent secondary procedures during follow up with secondary ptosis (18%) being the most frequent, followed by lipostructure (13%).
Conclusion
There were no differences among those three surgical techniques, providing various alternatives for managing a painful blind eye.
