Purpose

To describe the general characteristics, therapeutic management and outcome of patients diagnosed with Morbihan’s disease. It is indeed a rare pathology, leading to an erythematous non-pitting edema of the upper portion of the face

Methods

We reviewed all cases with a diagnosis of Morbihan’s disease at our center between 2010 and 2023.

Results

Ten patients were included, with a majority of men and a mean age at the beginning of symptoms of 53 years old. Most of our patients had a bilateral but asymmetrical presentation. The average delay between the first symptoms apparition and the diagnosis was of four years. Anatomohistopathology was performed and revealed lymphatic oedema in most of the cases. The majority of our patients had a systemic therapeutic (methotrexate, corticosteroids) induced after internal medicine consultation. Upper lid or lower lid blepharoplasty was performed, and intralesional corticosteroids were used in half of the cases. One patient had a supra clavicular graft at the same time. Post operative results were deemed satisfactory in most of the cases. One patient had a major edema in the early post operative stage, resolving following systemic corticosteroid treatment. Two patients had secondary relapse. One patient had a postoperative ectropion, treated by secondary skin grafts and lateral tarsal strip procedures.

Conclusion

Morbihan disease remains a therapeutic challenge, as it is a rare disease for which diagnosis is frequently delayed. Surgical treatment is complex, and relapses and complications are possible. Careful examination with internal medicine should be performed, as a need for systemic therapy should be assessed. Intra operative injection of corticosteroid seems to show satisfactory results.
