Purpose

To report the main diagnostic errors and describe mimicks in eyelid inflammatory and tumoral affections.

Methods

We retrospectively reviewed the charts of patients presenting with eyelid mass or eyelid inflammatory symptoms. We studied the first clinical diagnosis and compared it to final diagnosis. All patients presenting with mass had undergone surgical biopsy and had a histological diagnosis.

Results

Our serie included 253 patients presenting with eyelid masses and/or inflammatory symptoms. The initial diagnosis was incorrect in 25 cases (9,88%). Regarding clinical diagnosis of eyelid tumors, final diagnoses included sarcoidosis (n=4) , leshmaniasis (n=3), Histiocytoses (xanthogranulomatous disease: n=1, and Rosai Dorfman Disease n=1) and other tumors different from pre excisional diagnoses ( n= 16) like lymphoma, vascular malformation and adnexal tumors.

Conclusion

Eyelid affections are a very common reason for consultation in ophthalmology. Clinical presentation can be challenging, and final diagnosis may be different from clinical diagnosis, including tumors, inflammatory diseases. Oculoplastic surgeons and general ophthalmologists should be aware of most frequent mimicks to propose the best therapeutic strategy.
