Primary congenital bilateral upper eyelid retraction in an infant: a case report

Purpose

To report a case of primary congenital upper eyelid retraction.

Results

A four-month-old infant presented to our consultation for having larger eyes observed by their parents since birth. Pregnancy, birth, and development were normal. Clinical examination showed bilateral upper eyelid retraction with a 2 mm scleral show. No lagophthalmos or corneal anomalies were noted and fundus of both eyes were normal. Thyroid function tests including anti-TSH receptor antibodies and transfontanellar ultrasound revealed no underlying disease process. The diagnosis of primary congenital eyelid retraction was established, and we recommended monitoring. Six months later, the same bilateral eyelid retraction was noted.


Conclusion

Primary congenital eyelid retraction is a specific syndrome that can affect the upper eyelids, the lower eyelids, or both. It can only be diagnosed after exclusion of other causes of lid retraction. Among the most common factors are congenital hyperthyroidism, seventh cranial nerve palsy, trauma and hydrocephalus. Surgical repair may be indicated usually at the age of 4 years.
