Dermis Fat Graft for a Chronically Exposed Orbital Implant

Purpose
The purpose of this study is to present the surgical technique of harvesting a dermis fat graft, removal of a previous orbital implant, and implantation of the graft into the orbital cavity in an eye with chronic exposure of previous orbital implants.
Methods

Results
A 63-year-old woman underwent evisceration of the left eye due to a painful, non-light-perceiving eye. Two years after the evisceration, she was reevaluated and found to have conjunctival erosion with partial exposure of the implant. Palatal mucosal grafting was proposed to cover the implant, but it was ineffective, with total exposure of the ocular implant. Removal of the orbital implant and implantation of a dermis fat graft was then proposed.An oversized dermis fat graft was harvested from the buttock region to allow for post-operative shrinkage. The fully exposed implant was removed from the orbit, and the dermis fat graft was inserted. The fat graft was sutured to the conjunctiva, with the dermis exposed anteriorly and the fat tissue exposed to the orbit.Excellent post-operative aesthetic and functional outcomes were achieved at the 3-week follow-up. The graft was patent, and the plan is to order a customized ocular prosthesis to be placed over the graft.
Conclusion
The implantation of a dermis fat graft in eviscerated eyes achieves excellent post-operative results and flexibility in patient follow-up, particularly in those with multiple exposures of previous orbital implants.
