Ocular injuries associated with orbital fractures

Purpose
Determine the epidemiological, clinical and evolutionary characteristics and of ocular injuries associated with orbital fractures. – Identify prognostic factors in patients with orbital fractures. – Propose an appropriate strategy for the management of these injuries
Methods
we conducted a retrospective, descriptive study involving patients with ocular trauma associated with
orbital fractures. The study was conducted at the ophthalmology and maxillofacial surgery departments of
Tahar Sfar University Hospital in Mahdia, Tunisia, between July 2022 and December 2023.
Results
We included 58 patients. A corneo-scleral wound was found in three patients. Traumatic optic neuropathy was
diagnosed in 6 patients, superior orbital fissure syndrome (2 patients), Third nerve plasty (3 patients) and
globe rupture (3 patients). Orbital fractures involved a single orbital wall in 46.5% of cases. The anatomical
site most frequently affected was the orbital floor (41%), followed by the medial wall (21.5%), then the
lateral wall (20.5%) and finally the roof of the orbit (17%). The incidence of post-traumatic legal blindness was 15%. Poor visual prognosis factors were orbital trauma secondary to RTA, occurrence of initial loss of consciousness, low initial visual acuity (<1/10), presence of hemorrhagic chemosis, limitation of oculomotricity and impairment of afferent RPM. Radiologically, fractures with a poor prognosis were those of the medial wall and apex, fractures localized to a single orbital wall, the presence of orbital hematoma, and fractures associated with other brain lesions.
Conclusion
A complete ophthalmological evaluation, a maxillofacial examination and an orbital-brain CT scan are
recommended for all patients presenting with an orbital fracture.
