Recurrent Eyelid Lesions in Children: The Importance of Thorough Ophthalmological Assessment

Purpose

We present two cases that highlight the significance of a comprehensive ophthalmological evaluation in pediatric patients.

Results

Case 1: A 4-year-old child who presented with recurring styes on both eyelids. Upon conducting a slit lamp examination, the findings were largely normal. However, a significant hyperopia was noted with a refraction of +5 (-2 to 12) for the right eye and +4.5 (-2.25 to 160) for the left eye. A comprehensive treatment plan was developed, which included medical management and optical correction through glasses. Remarkably, the child showed signs of improvement relatively quickly, with no recurrence of the styes, thus preventing the development of amblyopia secondary to his refractive error.
Case 2: A 14-year-old patient presented with a 6-month history following the surgical excision of a chalazion on his right upper eyelid. He developed a gradually enlarging swelling at the excision site. Upon examination, a well-circumscribed, firm, superficial, and mobile mass was noted on the outer portion of the upper right eyelid, exhibiting a purple coloration. Fundoscopic examination was unremarkable, and systemic evaluation showed no abnormalities. Ultrasonography indicated the presence of a subcutaneous ovoid mass with a heterogeneous echostructure and mixed blood supply. Following this, total surgical excision was performed without complications. Histopathological analysis revealed nucleated basophilic cells at the periphery and islands of enucleated calcified ghost cells, accompanied by a foreign body reaction, leading to a definitive diagnosis of pilomatricoma.


Conclusion

These cases illustrate the necessity of detailed ophthalmological investigations for children with recurring eyelid lesions. Such assessments are crucial for accurate diagnosis and appropriate treatment planning. Early diagnosis and timely management can significantly reduce complications, enhance patient outcomes, and promote quicker recovery, ultimately improving the child’s overall well-being.
