Primary orbital lesions in Sweden- incidence and course

Purpose
To enhance our understanding of the incidence and clinical course of primary benign and malignant orbital tumours. This knowledge will contribute to improved management of these rare conditions by facilitating accurate diagnosis, and treatment in the future. We aim to:
• Determine the national incidence, age and gender distribution of primary orbital tumours from 2013 to 2023.
Methods
A retrospective chart review of patients with orbital diseases who were evaluated at St. Erik Eye Hospital – the only orbital center in Sweden- between 2013 and 2023. Patients were included if they had received one or more of the following diagnostic codes: H05.0–H05.9 (orbital diseases), C69.6 (malignant neoplasm of the orbit), or D31.6 (benign neoplasm of the orbit).
Results
Our study included a total of 415 patients, 2013-2023. The most common benign orbital lesion was Orbital Venous Malformation type 1, diagnosed in 112 patients (26.9%), with a median age of 56(23-86) years. The second most common benign lesion was various types of vascular malformations, found in 77 patients (18.5%), with a median age of 54(17-93). Pleomorphic adenomas were the third most common, identified in 17 patients (4.1%), , with a median age of 62(24-88) years. Other benign lesions were categorized as miscellaneous.
Among malignant orbital tumours, orbital lymphoma was the most common, diagnosed in 102 patients (24.5%), with a median age of 72(30-95)years. Further subclassification of these cases is pending. The second most common malignant tumour was optic nerve sheath meningioma, found in 24 patients (5.8%), with a median age of 52(27-93) years. Adenoid cystic carcinoma was the third most common, affecting 9 patients (2.9%), with a median age of 57(37-70). Other malignant lesions were classified as miscellaneous.
Conclusion
This is the first nationwide study of all primary orbital tumours. While underreporting of smaller and benign lesions is likely, this study provides a valuable foundation for further investigation into the prognosis and clinical course of orbital lesions.
