Late-Onset Contralateral Orbital Metastasis of Uveal Melanoma: A Case Report

Purpose
To present a rare case of contralateral orbital metastasis of uveal melanoma and to highlight its diagnostic and therapeutic difficulties.
Methods
A case of a 63-year-old patient with a history of right eye enucleation for choroidal melanoma in 2022 is described. Clinical course, imaging findings, surgical intervention, histopathology, systemic treatment, and follow-up are presented.
Results
After the enucleation of the right eye, the follow-up showed no signs of metastasis until late 2024, when the patient developed left-sided orbital symptoms. MRI suggested a benign lesion. Surgery was performed in December 2024 due to progression, optic nerve compression and visual obscurations. The in toto excision was not succesful. The patient was treated with combination immunotherapy (Yervoy/Opdivo) and stereotactic radiotherapy (CyberKnife). Follow-up imaging in May 2025 showed mild progression and lymph node involvement, with the patient tolerating treatment.
Conclusion
Contralateral orbital metastasis from uveal melanoma is exceedingly rare but increasingly recognized. It may manifest years after primary treatment and radiologically mimic benign lesions. This case supports the need for lifelong surveillance in patients with uveal melanoma and underscores the value of multidisciplinary management—including surgery, immunotherapy, and targeted radiotherapy—for improving local control and patient outcomes.
