Surgical excision and reconstructive repair of upper eyelid tumor following previous surgery

Purpose

To present the surgical management and functional reconstruction of an upper eyelid tumor in a patient with a history of previous eyelid surgery, focusing on oncological control and restoration of eyelid function.


Methods

A 58-year-old male patient presented with persistent redness, tearing, foreign body sensation, and corneal exposure due to upper eyelid architectural disruption following previous surgery. Clinical evaluation revealed significant lid malposition with associated exposure keratopathy. Histopathological analysis confirmed squamous cell carcinoma (SCC) with moderate differentiation and extensive keratinization, extending to the surgical margins, but without lacrimal gland involvement. The surgical approach included complete tumor excision to achieve clear oncological margins, followed by complex reconstructive repair. The reconstruction involved the use of local flaps and free skin grafting to restore upper eyelid structure and function, with particular attention to preserving corneal protection and optimizing cosmetic outcomes.


Results

Postoperative recovery was uneventful, with significant improvement in lid position, corneal protection, and patient comfort. Follow-up at six months confirmed stable reconstructive results, without evidence of tumor recurrence. The patient remains under regular monitoring for long-term oncological control.


Conclusion

Reconstruction following upper eyelid tumor excision presents unique challenges, particularly in cases with previous surgical history. Achieving both oncological clearance and functional restoration requires meticulous surgical planning and technique. This case highlights the importance of early intervention and long-term follow-up to prevent recurrence and optimize patient outcomes.
