Congenital anophthalmia treated with a dermis fat graft combined with a skin graft in the upper lid in the early childhood

Purpose

The objective of this report is to present three cases of children with unilateral true congenital anophthalmia (CA) and severely contracted sockets.


Methods

We report three cases of children with unilateral true congenital anophthalmia (CA) who presented with severely contracted sockets and reduced periorbital measurements. These patients underwent dermis fat graft (DFG) placement in the socket, combined with a skin graft to the upper eyelid, upon presentation to our service—two within their first month of life and one at seven months old. The DFG was harvested from the bottom region, and implanted into the socket, while the skin graft from its surface was applied to the upper eyelid. Postoperative management included the use of progressive conformers or external prostheses. A quantitative evaluation conducted six months post-surgery.


Results

A significant improvement in socket and periocular development was observed.
All three children successfully retained an external prosthesis.


Conclusion

The authors advocate for an early dermis fat graft (DFG) approach, combined with a skin graft to the upper eyelid and progressive conformer therapy, to expand the socket, increase eyelid dimensions, and enlarge the palpebral fissure. This strategy enhances prosthesis adaptation and improves the child’s overall facial appearance.
