Experience of Hyaluronic Filler (Restylane) for passive lagophthalmos in Paediatric Congenital Ichthyosis

Purpose
To investigate the effect on passive lagophthalmos (pLag) in paediatric congenital ichthyosis (PCI) patients, undergoing treatment with periocular Restylane injections (RI).
To assess parental/patient satisfaction with the treatment.
Congenital ichthyosis (CI) is an umbrella term used for a group of rare genetic conditions that cause severe drying of the skin, associated with shortening of the anterior lamellae and lid ectropion. Complications such as corneal ulcers and keratitis are common.
Methods
Data collection was completed retrospectively using case notes and images of patients attending the Oculoplastic clinic at Manchester Royal Eye Hospital from December 2018- May 2025.
Results
We investigated the case notes of 5 patients with CI who underwent RI therapy to all 4 lids. All patients presented with ectropion and pLag ranging from 4-6mm.
Ages during treatment ranged from 17 months to 14 years. RI were delivered sub-orbicularis under general anaesthesia, 0.25-1mL per lid.
All patients treated note subjective improvements at 6 week followup. 3/5 patients achieved 0-1mm pLag. 2/5 patients note initial improvement of 1-2mm pLag, but return to initial lid closure over time. 100% of families expressed desire over the long term to continue treatment. One child only required observation after 2 treatments due to successful outcome.
Conclusion
RI treatment provides a minimally invasive way of improving or stabilising pLag in patients with CI, with patient satisfaction that would encourage families to carry on with the treatment. Frequency of treatment is tailored to each patient, but multiple injections may be required to maintain an adequate effect.
This method of anterior lamella expansion avoided the need for full thickness skin grafts in this cohort.
