Lantern of the Eye or the glowing eyelids: A case report.

Purpose
To report a periocular apocrine hidrocystoma case treated surgically.
Methods
A 40-year-old female presented with a right eye periocular cystic lesion that has spontaneously appeared and progressively grown over the last year. The patient underwent slit-lamp examination, surgical excision, and pathological examination.
Results
Right eye examination showed a solitary well-circumscribed, mobile, translument cystic lesion, measuring 2 x 2 centimeters and appended to the lateral canthal skin. The cyst was overlayed by thin blood vessels. Ophthalmic evaluation was otherwise normal in both eyes. The diagnosis of an apocrine hidrocystoma was highly suspected. A surgical excision followed by histopathological examination was performed, both for aesthetic purposes, and to rule out malignancies. Pathology results confirmed the diagnosis and found no signs of dysplasia. Post-operative follow-up of 6 months found no recurrences.
Conclusion
Periocular apocrine hidrocystomas are uncommon benign cystic lesions that originate from apocrine sweat glands. Their pathophysiology remains unknown. These lesions are most frequently found among individuals aged 30 to 70 years, with no sex predisposition. The diagnosis is easily suspected clinically. However, since these lesions can mimic cutaneous neoplasms such as basal cell carcinomas, surgical excision and pathological evaluation are necessary.
