Optimising Oculoplastic Theatre Efficiency at Milton Keynes University Hospital

Purpose

Theatre availability across the NHS is under increasing pressure, making it essential to maximize efficiency and minimize cancellations. Operating theatres are costly to run, and same-day cancellations result in wasted resources and reduced service capacity. In our oculoplastic theatre list, most procedures are performed under local anaesthetic, meaning patients do not currently undergo a formal pre-operative anaesthetic assessment. However, cancellations and non-attendance—often due to medical issues or inappropriate listing—have impacted efficiency. This audit aimed to assess the extent and causes of cancellations and implement measures to minimise their occurrence.


Methods

Data was collected from August 2024 to January 2025, analysing attendance rates, procedure changes, and cancellations. Additionally, we evaluated the need for supplementary measures such as anaesthetic support and pre-operative assessments to improve theatre session efficiency.


Results

Of the 52 patients listed during the audit period, 7.6% had same-day cancellations, primarily due to medical issues such as hypertension and pyrexia. Additionally, 3.8% did not attend their scheduled surgery.


Conclusion

Our findings highlight the need for pre-operative assessments to identify patients with high-risk medical comorbidities and ensure they are fit for surgery. To further reduce cancellations, we have also introduced a pre-theatre telephone confirmation process. These interventions aim to improve theatre utilization and enhance the overall efficiency of oculoplastic surgical services.
