Corneal Sensitivity in Ectropion and Entropion: Postoperative Recovery and Clinical Implications

Purpose
To assess corneal sensitivity (CS) in patients with ectropion or entropion before and after eyelid surgery, and explore associations with topical treatments, comorbidities, prior ocular surgery, dry eye severity (Oxford scale), and tear break-up time (TBUT). The aim is to determine whether CS, measured via portable esthesiometry, may serve as a useful parameter in surgical decision-making.
Methods
Prospective study using the Brill Esthesiometer, a non-contact device delivering air pulses (2–10 mBar, 0.4 s) at a fixed distance (4 ± 1 mm) guided by infrared LEDs. CS was graded from 1 (hypersensitivity) to 6 (very severe hyposensitivity, no stimulus perception). Additional variables included TBUT, Oxford scale, use of topical medication, systemic comorbidities, and ocular surgical history.
Results
Preoperative CS was commonly altered in both entropion and ectropion. Postoperatively, a clear trend toward normalization was observed across cases. CS alterations were more frequent in patients with diabetes or autoimmune disease, and showed no evident association with prior ocular procedures.
Conclusion
Postoperative improvement in CS supports the hypothesis of a reversible neuroepithelial dysfunction in eyelid malpositions. These findings reinforce the potential of corneal sensitivity as a prognostic biomarker in oculoplastic surgery. Ongoing data collection will help validate its role in clinical decision-making.
