Platelet‑rich plasma as a novel therapy for post‑traumatic periorbital scars

Purpose
To evaluate the clinical and sonographic effects of serial intralesional platelet‑rich plasma (PRP) injections on post‑traumatic periorbital scars.
Methods
Fifty adults (35 – 45 years) with established traumatic periorbital scars received nine weekly intralesional PRP treatments. Scars were reviewed every three weeks and again three months after the final session. Outcomes were: (1) high‑frequency ultrasonography for vascularity and echogenicity; (2) digital colour analysis; (3) a validated questionnaire on tightness during facial expression; and (4) adverse‑event monitoring.
Results
After three PRP sessions, increased microvascular flow was demonstrated sonographically in 35/50 participants (70 %). By session six, echogenicity had fallen in 32/50 (63 %), indicating fibroblast activation and collagen remodelling. Three months post‑treatment, mean scar redness had declined by 20 % in 38/50 patients (75 %), while 29/50 (58 %) reported less contracture during mimetic movement. No infections, pigmentary change or visual complications were observed.
Conclusion
Serial intralesional PRP appears to remodel post‑traumatic periorbital scars, enhancing vascularity, reorganising collagen and improving colour and pliability without significant complications. Controlled trials with longer follow‑up are recommended to confirm efficacy and optimise dosing protocols.
