High risk nodular Basal Cell Carcinoma with perineural extension post an episode of acute contact dermatitis

Purpose

To report a case of a high risk nodular infiltrative Basal Cell Carcinoma with perineural extension that developed after an acute episode of allergic contact dermatitis.


Methods

Review of patient’s notes including clinical examination, histopathology findings, external photographs, radiological imaging and management.


Results

71 years old male presented with history of acute episode of allergic contact dermatitis in the right temple / lateral canthus area and cheek after exposure to Foxglove plant sap, this was treated by his GP. Patient subsequently developed a gradually enlarging lesion in the right temple / lateral canthus where the allergic reaction happened. The lesion was excised in a staged approach / delayed MOHS and histopathology showed a high risk nodular infiltrative Basal Cell Carcinoma with deep invasion into temporalis muscle and perineural extension. Further excision achieved clear margins and MRI scan ruled out deeper tissue and perineural invasion.


Conclusion

To our knowledge, there was no previous case report of Basal Cell Carcinoma developing after an acute episode of contact dermatitis. A previous systematic analysis concluded that Chronic Atopic Dermatitis is associated with higher risk of non-melanoma skin cancers like BCC and SCC, the mechanism is still unclear and requires further studies and investigation.
