Ocular complications of facial fillers and botulinum toxin injections: A comprehensive systematic review

Purpose
To review the ocular complications of facial fillers, soft tissue, and botulinum toxin injections.
Methods
The review protocol was pre-registered on PROSPERO [CRD42023442103]. PubMed, Scopus, Web of Science (WOS), CENTRAL, and Google Scholar were searched from inception until December 30, 2023. The primary outcome included early and late ocular complications of facial fillers andor botulinum toxin injections. The Social Sciences Statistical Program (SPSS) was used for the statistical analysis. The risk of bias was assessed using the National Institute of Health tool.
Results
A total of 86 studies with a total number of 356 cases were included. The patients’ ages ranged between 18 and 83, with a mean (SD) age of 44.9 (14.42). The most received injection was hyaluronic acid (HA) representing (n=148) 41.6%, followed by botulinum toxin which represented (n=71) 19.9%, and 84 (23.6%) patients received fat. Ptosis was reported in 113 cases (31.74%), pain in 90 (25.28%), and diplopia in 70 (19.66%). Females were associated with higher risks of developing these complications (P value < 0.001). Vision impairment or blindness was developed in 164 cases (46.06%) with no significant difference between males and females (P value = 0.185). Age was not significantly associated with diplopia (P value = 0.639).
Conclusion
Lower orbital or periorbital swelling and masses were observed more with facial injections compared to upper. About one-third of the patients experienced ptosis. Also, nearly one-half of the patients experienced impairment in their vision or blindness. These complications were observed more in females and old-age patients.
