Unusual Ocular Sarcoidosis Manifestations

Purpose
We present two cases of unusual ocular lesions indicative of systemic sarcoidosis in the same family.

Results
Case 1: A 20-year-old female presented with ocular swelling lasting two weeks . Upon examination, the right eye exhibited conjunctival hyperemia, multiple sub-conjunctival whitish nodules. Retro corneal precipitates were notified bilaterally. Fundus examination revealed diffuse white lesions in the peripheral retina. A fluorescein angiography was performed, showing multifocal peripheral choroiditis in the right eye. The left eye displayed conjunctival hyperemia and well-defined nasal granuloma extending to the limbus. An etiological workup revealed hypercalcemia, hypercalciuria, elevated angiotensin-converting enzyme levels, and interstitial lung syndrome on chest radiography. These findings culminated in a diagnosis of sarcoidosis. The patient was treated by local corticosteroid therapy with regression of granuloma.
Case 2: A 22-year-old sister, presented with a recurrent conjunctival lesion that had been surgically removed one year earlier, initially misdiagnosed as pterygium. After the surgery, she experienced a recurrence at the same site, presenting as localized conjunctival swelling and mild discomfort. An etiological workup conclude to the diagnosis of sarcoidosis.
Conclusion
These cases illustrate that conjunctival granulomas may be an ocular manifestation of sarcoidosis, highlighting the importance of not misdiagnosing them as pterygium. Additionally, sarcoidosis has genetic component. It is crucial to consider sarcoidosis in the differential diagnosis of ocular swellings, even in the absence of evident systemic signs.
