Upper entropion resolution by GVHD using Cuenod-Nataf technique

Purpose
Upper eyelid entropion is caused by alterations in the posterior lamella of the eyelid due to conjunctival abnormalities. The most common cause is trachoma, but other pathologies can also cause it, such as graft-versus-host disease (GVHD), which can be treated with trachoma surgical techniques.
Methods
A 56-year-old male patient was diagnosed with Mycosis Fungoides 16 years ago.He received nine lines of oncohematologic treatment between 2008 and 2021, the last being a haploidentical allogeneic transplant (allo-HSCT). In 2024, he developed GVHD with skin, kidney, and eye involvement.He presents symptoms of severe dry eye and entropion of the upper eyelid of the left eye. He underwent entropion surgery using the modified Cuenod-Nataf technique in May 2024. Despite ophthalmic treatment, he developed entropion of the upper eyelid of the upper eye four months later and underwent surgery using the same technique in December 2024. In the procedure, we made an incision in the gray line of the eyelid margin, releasing the tarsus, creating a tarsal fracture, and passing three mattress sutures from the free edge to the tarsus.
Results
Entropion has been resolved in both eyes, with a follow-up of 12-6 months, without trichiasis, keratitis or lagophthalmos. He continues with severe dry eye treatment for his underlying disease.
Conclusion
GVHD can cause severe dry eye and entropion of the upper eyelid.Treatment is directed at the etiology of the condition and the treatment of dry eye. When there is structural damage to the eyelid, surgery is necessary to address it.Surgical techniques, which may be obsolete in our setting, must be revived to ensure proper treatment.In hematological patients with ocular symptoms, multidisciplinary management is required to provide a proper diagnosis and treatment.
