A Retrospective Study of Cryotherapy for Basal Cell Carcinoma of the Eyelids in Finland

Purpose
To evaluate the effectiveness and side effects of cryotherapy for treating nodular and superficial basal cell carcinoma (BCC) of the eyelids.
Methods
In this retrospective study, we analyzed the outcomes of cryotherapy for primary nodular and superficial BCC of the eyelids, treated at the Department of Ophthalmology, Helsinki University Hospital, from 2009 to 2023. Inclusion criteria included biopsy-proven BCC with distinct borders and a minimum follow-up of 18 months. Treatment involved double freeze-thaw cycles using liquid nitrogen with a closed probe. Follow-up visits were conducted at 6, 18, and 30 months.
Results
A total of 49 patients were included in the study. Almost all (94%) of the tumors were situated on the lower eyelid. Most (67%) tumors were staged as T2b, and the median tumor size was 6 mm. The median follow-up time was 2.6 years. No recurrences were observed. Side effects included loss of lashes (100%), depigmentation (55%), ectopic conjunctiva (53%), and punctal eversion (6%), with most side effects being mild.
Conclusion
Cryotherapy is an effective and safe alternative to surgery for low-grade BCC of the eyelids, offering a less invasive and cost-effective treatment option compared to surgery. Elderly frail patients and those with tumors close to the lacrimal duct can especially benefit from cryotherapy instead of surgery. Proper training and adherence to treatment protocols are essential for ensuring its effectiveness.
