Efficacy and Safety of Intralesional Bleomycin in the Treatment of Orbital Lymphaticovenous Malformations

Purpose

To evaluate the efficacy and safety of intralesional Bleomycin injections in the management of orbital lymphaticovenous malformations (LVM) at King Khaled Eye Specialist Hospital (KKESH), focusing on lesion size reduction, symptom relief, recurrence, and complication rates.


Methods

To evaluate the efficacy and safety of intralesional Bleomycin injections in the management of orbital lymphaticovenous malformations (LVM) at King Khaled Eye Specialist Hospital (KKESH), focusing on lesion size reduction, symptom relief, recurrence, and complication rates.


Results

:
The cohort included 12 females (66.7%) and 6 males (33.3%) with a mean age of 8.2 ± 8.1 years (range: 0.6–27.8 years). Periocular swelling (55%) and unilateral proptosis (44%) were the most common presenting signs. Lesions were classified as microcystic (16%), macrocystic (55%), or mixed (27%), with 61% demonstrating both intra- and extraconal involvement. The mean initial lesion area was 671 ± 375 mm², significantly reduced to 279 ± 280 mm² at final follow-up (mean follow-up: 78.6 ± 96.6 months). The mean overall size reduction was 61%, with complete resolution observed in 11.8% of patients. Among the five patients who initially presented with an afferent pupillary defect, four (80%) demonstrated complete resolution following treatment. No cases of lesion recurrence were observed. Minor complications occurred in 27% of patients, primarily periocular swelling, with no major systemic adverse events recorded.


Conclusion

:
Intralesional Bleomycin injections provide a safe and effective treatment option for orbital lymphaticovenous malformations, achieving significant lesion size reduction, improvement in afferent pupillary defect, and symptom relief with minimal complications and no recurrence over long-term follow-up.
