Purpose

To evaluate the effectiveness of brow lift surgery combined with upper eyelid skin excision in the treatment of severe dermatochalasis.


Methods

This interventional, descriptive, non-controlled study was conducted on 40 eyes diagnosed with severe upper eyelid dermatochalasis associated with brow ptosis. All cases underwent surgery at the Vietnam National Eye Hospital between January 2023 and December 2024. Inclusion criteria involved patients with functional or cosmetic complaints related to excess upper eyelid skin and brow descent. Preoperative assessment included photographic documentation, measurement of eyelid crease height, and classification of dermatochalasis severity. The surgical technique involved simultaneous brow lift and upper eyelid skin excision tailored to individual anatomical characteristics. The minimum follow-up duration for all patients was 6 months. Postoperative outcomes were evaluated based on eyelid crease height improvement, reduction of periorbital wrinkles, patient satisfaction, and complication rates.


Results

The male-to-female ratio was 3:7, and 60% of patients were over 60 years old. Functional and cosmetic concerns were the primary reasons for surgery in 65% of cases. Among the 40 eyes, 70% were classified as grade 2 dermatochalasis and 30% as grade 3. The mean eyelid crease height increased from 3.25 ± 0.18 mm before surgery to 5.15 ± 0.12 mm after surgery. All patients (100%) experienced visible improvement in lateral canthal wrinkles, including both static and dynamic lines. Overall satisfaction was high, with 90% of patients reporting they were very satisfied and 10% satisfied. The procedure had a success rate of 95%, with no major complications observed during the follow-up period.


Conclusion

Brow lift surgery combined with upper eyelid skin excision is an effective and safe treatment modality for patients with severe dermatochalasis and brow ptosis. It provides both functional and aesthetic benefits, enhances upper eyelid contour, and achieves high levels of patient satisfaction.
